Problems are first seen by whoever does the work: a leak, a missing spare part, a machine that sounds different. They tell the supervisor in person, at the shift meeting or on a note on the board, and the analyst moves it into a list. What is said in the hallway does not arrive, what arrives without a photo or a place cannot be understood, and whoever reported it never hears what became of it, so they stop reporting.
